Named Entrance Awards Application Form

In preparation for completing this form have the following documents ready in DOC or PDF format:

  • A letter of recommendation sent from high school Principal, Guidance Counsellor, or Teacher. (You can attach the letter to the form or have your referee send it directly to entranceawards@)
  • A resume, which should include such things as involvement in community service, participation in student government, athletic achievement, extra-curricular activities and volunteer and/or paid work.
