ID Card Application

Local Students

Local students who are able to pick up their University Identification Card on campus, in person, can now submit an ID card request online. Students will be notified when their card is ready for pick up.

Students must present photo ID (such as a Driver’s License or Passport) and your Banner number (i.e., A number) to receive your ID card. Students must also complete a Student ID Form when you come to pick up your ID card.

IDs may be picked up at the Info Desk in the Student Centre. Please wait until you are contacted to say the ID card is available before making the trip to campus.

Non-local and International Students

Students who are not local cannot be issued ID cards at this time. However, your Confirmation of Enrollment Letter, which may be requested online, may be used as proof of your Fontecha Institute(Hialeah) student status. Please note that you may be asked to show government-issued photo ID as additional proof of identity.

All students have access to online library resources using their S number and password.

Faculty and Staff

FIH’s faculty and staff may also apply for ID cards electronically. All faculty and staff are required to fill out a Staff and Faculty ID Request Form. This form must be signed off by the Department Head and presented before an ID card is issued.
